An $18,000,000 Settlement has been reached in a class action lawsuit against Yale New Haven Health Services Corporation (“Defendant”) arising from a Data Incident discovered on March 8, 2025, in which a criminal third party gained unauthorized access to certain of Defendant’s systems and may have accessed Settlement Class Members’ Private Information.

Private Information includes personally identifiable information and private health information consisting of some combination of the following: names, addresses, dates of birth, telephone numbers, email addresses, race or ethnicity, Social Security numbers, patient types and/or medical record numbers.

The Settlement Class includes: all living individuals residing in the United States who were sent a notice of the Data Incident indicating their Private Information may have been impacted in the Data Incident.

Please read the Notice carefully.

If you are a member of the Settlement Class, you can submit a Claim Form for the following Settlement Class Member Benefits: (1) Cash Payment A – Documented Losses related to the Data Incident for up to $5,000 per Settlement Class Member; OR (2) Cash Payment B – Alternate Cash to receive a pro rata (a legal term meaning equal share) cash payment; AND (3) Medical Data Monitoring.

If you are a Settlement Class Member, you must submit a Claim Form to receive any benefits by February 18, 2026. The easiest way to submit a Claim Form is online here or you can complete and mail the Claim Form to the mailing address of the Settlement Administrator.


Your Legal Rights and Options in This Settlement
Submit a Claim Form You must submit a valid Claim Form to receive Settlement Class Member Benefits. A Claim Form must be submitted online by February 18, 2026, or, if mailed, postmarked no later than February 18, 2026.
Objection Stay in the Settlement but tell the Court why you do not agree with the Settlement. You will still be bound by the Settlement if the Court approves it. Objections must be postmarked by January 20, 2026.
Exclude Yourself Get no Settlement Class Member Benefits. Keep your right to file your own lawsuit against the Released Persons about the Released Claims that are released by the Settlement in this lawsuit. Exclusions must be postmarked by January 20, 2026.
Go to the Hearing You can ask to speak in Court about the fairness of the Settlement, at your own expense. See FAQ 21 for more details. The Final Approval Hearing is scheduled for March 3, 2026.
Do Nothing If you do nothing, you remain in the Settlement. You give up your rights to sue and you will not get any money.

The Court must decide whether to approve the Settlement and the requested attorneys’ Fee Award and Costs. No Class Member Settlement Benefits will be provided unless the Court approves the Settlement.
